Parent + Student Contract- Please Read
Parent Release Form for Media Recording
I, the undersigned, do hereby grant or deny permission to VOCAL MOTION MUSICALS to use the image of my child, Such use includes the display, distribution, publication, transmission, or otherwise use of photographs, images, and/or video taken of my child for use in materials that include, but may not be limited to, printed materials such as brochures and newsletters, videos, and digital images such as those on the VOCAL MOTION MUSICALS Website.
I give unrestricted permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by VOCAL MOTION MUSICALS for a variety of purposes and that these images may be used without further notifying me. I do understand that the child’s last name will not be used in conjunction with any video or digital images.
Release and Indemnity
IN CONSIDERATION OF being permitted to participate in VOCAL MOTION MUSICALS, the undersigned, on behalf of myself and the participant.
1. Acknowledges that the child’s participation in VOCAL MOTION MUSICALS performing group may include activities that may be hazardous to the participant and assumes the risk of injury or harm associated with such participation.
2. Releases and forever discharges VOCAL MOTION MUSICALS and its employees, directors and representatives, (collectively the "Releasees") of and from all liability, claims, demands, damages, costs, expenses, actions and causes of action (collectively the "Claims") in respect of death, injury, loss or damage to myself or property howsoever caused, arising or to arise by reason of or during my participation in the Event, whether prior to, during or subsequent to my attendance and notwithstanding that any Claim may have been contributed to or occasioned by the negligence of any of the Releasees.
3. Indemnifies and holds harmless the Releasees from and against any and all liability incurred by any or all of them arising as a result of or in any way connected to my participation in VOCAL MOTION MUSICALS.
4. Understands and acknowledges that VOCAL MOTION MUSICALS does not carry or maintain health, medical or disability insurance coverage for the undersigned and therefore agrees to assume responsibility for such insurance coverage on the undersigned.
5. Agrees that in the event that any provision of this Release and Indemnity is held to be invalid or unenforceable by any court of competent jurisdiction, the invalidity or unenforceability of such provision will not affect the remaining provisions of this Release and Indemnity, which shall continue to be enforceable.
Attendance Policy
Consistent attendance is essential to the success of our production and the growth of each student. Because rehearsals build progressively, missed time affects not only the individual student but the entire cast.
Known Conflicts
If you have a known conflict, please notify us as soon as possible so we can plan accordingly.
Unexcused Absences
As outlined in the audition contract, excessive unexcused absences may result in a student losing their role.
Dress Rehearsals & Performances
All dress rehearsals and performances are mandatory. Because every student plays an important part in the production, attendance at these events is required for participation in the show.
We appreciate your partnership in helping your student stay committed and present throughout the season. Thank you for supporting their growth and the success of our production!
Student Contract
I will be positive, cheerful, and uplifting! I will respect my fellow cast members and my teachers. I will be present for all rehearsals. If I have three unexcused absences, I understand that my role may be reassigned to another student. If I am given a script, I will take care of it and bring it to every rehearsal. I am required to return the script in excellent condition, and if I don’t, I will be charged an additional fee.
I promise to practice my lines, music, and dancing so that when I come to rehearsal, I am prepared!
I pinky promise that I will have loads of fun and make lots of AWESOME memories!!
